To provide a template that can be used to capture issues in general, prioritize them and track them. While this template was created for use during onboardings, it can be used in other situations as well.

1. As you go through the onboarding process and identify items that need to be addressed in the client environment such as:
2. Replacing a server
3. Configuring guest wireless
4. Upgrading an application
5. Use the attached document to write down what the issue is, the risk to the business
6. Either during and/or at the completion of the onboarding process IT Manager and CIO should review the document, prioritize the issues, review with the client to determine next steps and ownership of the issue
